This study explored Dignity Therapy as a concept and also its potential impact on existential attitudes about living and dying among a generally healthy, older population. Dignity therapy is a brief, novel, individualized intervention created by Harvey Max Chochinov for use with the older, imminently dying population and the terminally ill populations to address psychosocial and existential distress (Chochinov, Hack & Hassard, 2005). This humanistic, narrative intervention comprises individual sessions involving a trained interviewer and a participant; its intent is to encourage the participant to reflect on salient life memories and how they want to be remembered. The overarching goal is to inspire an individual's sense of meaning and purpose in life. Sessions are recorded, transcribed and edited, and then provided to the individual in the form of a written document that can be shared with a friend or family member. This study explored participants' experiences of Dignity Therapy, and whether Dignity Therapy elicited reflection on ideas related to existential themes - in particular, ones related to self, life purpose and satisfaction, and attitudes about death. Qualitative analysis revealed significant recurring themes across participants. The process of Dignity Therapy allowed for the discussion of values regarding life meaning/satisfaction, integration and acceptance of negative experiences into one's life narrative, reaffirmation of one's choices and beliefs, and new insights about one's life. Participants observed greater levels of life satisfaction, a more cohesive and integrated life narrative, and improved self-esteem. The experience of Dignity Therapy elicited thoughts about the end-of-life stage, including death and the possibility of an afterlife, bringing about a greater sense of death acceptance. Participants shared similar concerns about how their family members might feel about receiving the Generativity Document. Additionally, participants reported concerns about what was said or not said in the final Generativity Document. Important aspects of care tenor were identified that promote a helpful Dignity Therapy experience, such as a non-judgmental stance and positive regard for the participant. These emergent themes, and the ramifications of these findings on the feasibility of Dignity therapy as a helpful intervention with generally healthy, older adults are discussed.
