This study investigated the relationship between cognitive changes and Transcranial Magnetic Stimulation therapy. Three patients with Treatment Resistant Depression or TRD completed the IVA-2 Continuous Performance Test measuring 9 cognitive domains (focus, mental concentration, speed, agility, presence, stamina, steadiness, self-control and resilience); and the Beck Depression Inventory or the BDI-II to measure changes in depression.

This study tested a series of hypotheses using a Single Subject Experiment Design or SSED with the ABA structure (Baseline, Treatment & Withdrawal phases) with Tau-U analysis to examine a) if positive cognitive changes were associated with TMS therapy, and b) if there is a delayed antidepressant effect with TMS measured by changes in cognition, and c) if an inverse positive relationship was found between TMS treatment and depression, and d) if an inverse negative relationship existed between cognitive changes and depression scores, and e) can Tau-U analysis detect cognitive changes using the SSED utilizing the IVA-2.

Tau-U analyses did not support the correlation between positive changes and TMS therapy or if there was a delayed antidepressant effect. Furthermore, the data did not support an inverse relationship between TMS and Depression scores; did not support an inverse negative relationship between cognitive changes and depression. The Tau-U appeared to detect cognitive change, but these changes could not be attributed to the effects of TMS. The implications for counselors, practitioners and implication for the field are discussed, and recommendations for future research are provided.
