Background. Research has shown that once an ankle sprain has occurred, at least 80% of those have developed chronic ankle instability. There has been an abundance of research on balance training for this population but limited research on hip strengthening and its effects. Purpose. The aim of this study was to demonstrate the effects of hip strengthening on a patient with chronic ankle instability compared to traditional interventions. Case description. The patient was a 26-year-old male with a history of multiple bilateral ankle sprains and complaints of left more than right ankle pain and instability with jogging and participating in work duties. The intervention included hip strengthening exercises for four weeks and the patient demonstrated significant improvements in balance and perceived stability. Literature review. Current literature has supported the relationship between decreased hip strength and activation in patients with chronic ankle instability. Research has been limited in the effectiveness of a hip strengthening protocol for this population. Discussion. In this case, hip strengthening was an effective treatment to improve static and dynamic balance and perceived stability in a patient with chronic ankle instability. From the current research and this study, hip strengthening may be a key component to improving function in this population. It is important to educate patients and therapists on the benefits of hip strengthening to help improve activities of daily living in patients with chronic ankle instability.
