Background: Anterior knee pain affects up to 2.5 million runners annually. Runners with anterior knee pain alter joint motion to reduce pain over the course of a run, leading to altered force production and patellofemoral joint mechanics. Fatigue plays a role in these kinematic changes, though the effects on muscle force production remain unknown. Purpose: To examine the effects of pain and fatigue on muscle force production during a prolonged run for female runners with chronic anterior knee pain. Methods: Female runners age 18-45 with anterior knee pain for 3 months who run at least 9 miles per week were recruited. Pre and post run strength testing and quiet standing were performed. Electromyography (EMG), pain, and rated perceived exertion (RPE) were recorded at the start, middle, and end of a 3 mile run. Results: EMG did not yield significance over the course of the run between subjects; however, there were some insignificant trends found between the hamstrings and quadriceps. HR increased in the presence of pain in all 3 subjects. High correlation was seen between RPE and HR at the beginning of the run, with the correlation decreasing towards the end of the run. Conclusion: There is a relationship between co-contraction of the hamstrings and quadriceps in runners with knee pain, though it is unknown whether it is an adaptation to pain or causative. A linear relationship between pain and HR over the course of the run was also found. Further investigations are needed to connect kinetic changes to changes in muscle force production in the development and chronicity of anterior knee pain.
