Heart failure (HF) is a chronic and public health concern with increasing prevalence in the United States. A low-sodium diet is often recommended for HF patients to help manage symptoms. Few questionnaires exist to help clinicians gain insight into reasons why sodium adherence is often poor. Therefore, the purpose of this study was to develop and test a valid, feasible, and reliable questionnaire to assess low-sodium diet knowledge and stage of change in heart failure patients. A Sodium Assessment Questionnaire was developed with two constructs; knowledge and stage of change. The questionnaire was sent out to 11 HF experts to assess content validity. A sample of HF patients was used to assess face validity and feasibility (n=16). Another convenience sample of 100 HF patients were used to assess internal consistency and knowledge and stage of change of these patients. The questionnaire was deemed valid, with 70% agreement amongst content experts. Face validity was obtained (100% agreement; n=16). The questionnaire was deemed feasible (<10 minutes to complete; n=16). Reliability of responses from knowledge and stage of change domains was 0.76 (KR20) and 0.80 (Cronbach's alpha), respectively. The median (IQR) knowledge score was 5 (4, 5) on a scale of 0 to 7, and stage of change was associated with specific sodium-related behaviors (rho 0.10-0.43). A valid, feasible, and reliable questionnaire about the low-sodium diet was created, which will enable dietitians to assess reasons for poor adherence to low-sodium diets by HF patients.
