The purpose of this study is to evaluate the efficacy of a multifaceted nurse-coaching via telephone calling intervention over a three months period to help diabetic patients self- manage their disease to avoid complications. The intent of the nurse-coaching intervention is to (a) provide education and educational reinforcement; (b) assist patients in integrating dietary and exercise treatment recommendations in their daily lives; and (c) provide psychosocial support for changing roles, relationships, and emotions. A total of 39 participants were recruited from Sacramento Community Clinic. Data was collected on physiologic variables (Hemoglobin A1C, BMI) and psychological variables (Problem Areas in Diabetes [PAID]) (Polonsky, Anderson, Lohrer, Welch, Jacobson, Aponte, and Schwartz, 1995). First, descriptive information about the demographics of the sample were calculated. Next, frequencies and descriptive statistics were examined at pre- intervention and also at post-intervention. Finally, a paired-sample t-test was used to compare the pre and post intervention scores for all variables. It was predicted that patients would be able to reduce their hemoglobin A1C level by 10% from their original levels after the 12 weeks of intervention. Though BMI did not significantly decrease from pre-intervention to post-intervention, both PAID scores and AIC levels significantly decreased after the intervention, in support of the hypothesis. This study provides preliminary evidence that the multifaceted nurse-coaching via telephone calling intervention motivates participants to increase compliance with health-promoting behaviors, move toward a target goal of hemoglobin A1C <7.0, and be able to self adapt with the psychological issues that accompany chronic disease.
