Background: Changes in the health care environment have increased the demand for primary care providers and increased opportunities for independent practice. Continued opposition to independent practice has heralded Nurse Practitioners (NP) to define their role in health care and establish their capabilities. Given the overall shortage of nurses in the United States, unfavorable work environments could discourage nurses from pursuing advanced practice education, which could limit positive work environments that NPs offer as health care providers. As the provisions in the 2010 Patient Protection and Affordable Care Act, (Medicaid, n.d.) unfolds, many states are looking at creative strategies for improvement and lowering health care costs.

Purpose: The purpose of this qualitative phenomenological study was to explore the lived experience of Nurse Practitioners in independent practice. The aim was to give NPs a voice to express their individual experience, provide an inductive description of the lived experience, and gain understanding of the essence of being an independent practitioner. Philosophical Underpinning: This qualitative study was guided by the hermeneutic phenomenological approach of Max van Manen (1990).

Methods: A qualitative methodology utilizing the tradition of phenomenology as its basis was employed in this study. Data were obtained from a purposive sample of 17 Nurse Practitioners in independent practice.

Results: From the experiences of the 17 NPs in independent practice, four core and sub themes were identified.

Conclusions: Nurse practitioners in this study assessed the changing health care market environment and found a way to improve personal and functional outcomes. They developed a creative process of carving out a small part of the market by aligning their unique skills to match the needs of a population whose health care needs were not being fulfilled. NPs in this study viewed having total control over their practice as a necessary component if they were to step out as an alternative provider of health care. Their determination and professional confidence in the desired outcome paid off with the final result leading to a successful practice and a sense of achievement.
