This dissertation examines the effect of the National Health Insurance and female autonomy on neonatal mortality in Ghana. The dissertation consists of three chapters. The first chapter investigates whether the national health insurance has any impact on the risk of neonatal deaths using data from the 2008 Ghana Demographic and Health Survey. Using the same data, chapter two examines the effect of female autonomy in household decision making on the probability of neonate survival. Chapter three also explores the spatial analysis of the determinants of national health insurance enrolment decisions in Ghana.

Chapter one examines the effect of the national health insurance on the risk of death of babies within the first month of life using the 2008 Ghana Demographic and Health Survey. The analysis uses the probit model estimation due to the binary nature of the response variable. The results find no significant effect of the national health insurance on the risk of neonatal death. The results also show that, educated women take better advantage of the health insurance than uneducated women.

Chapter two examines the whether female autonomy in making household decisions has an influence on the risk of child survival. This chapter employs the principal component analysis as well as a probit model. The results find no consistent effect of female autonomy on child survival in Ghana. On one hand, autonomy in health decisions significantly influences child health while autonomy in financial decisions does not have significant effect on child health. Results from the principal component analysis also show a mixed result. When factors are considered separately, the estimates show a negative effect of autonomy while both factors considered jointly suggests otherwise.

Chapter three examines the determinants of national health insurance enrolment using spatial analysis. In addition to the OLS, I estimate an individual level probit analysis which considers the average NHIS enrolment of everybody else. The results suggest that, at the micro level, decisions to enroll onto the scheme are influenced by the average enrolment decisions of other neighboring women. Similarly, the results from the spatial lag model suggest the influence of social networking on the decision enrolment of women in Ghana.
