Many screening tools are available for use in hospitalized patients; however, no gold standard currently exists. The purpose of the study was to determine validity, reliability, and feasibility of a new tool, including functional status, oral intake, muscle wasting, and SIRS (FORMS). Adult patients admitted to general medical floors were included. Criterion validity was assessed using sensitivity and specificity between FORMS and Subjective Global Assessment (SGA). Differences in albumin and LOS were used to provide evidence for construct validity. A total of 213 patients were included; 94 at risk and 64 malnourished. Sensitivity and specificity of FORMS were 89.0% and 80.0% respectively. At risk patients (n=70) had a lower serum albumin (n=60) (3.2+/-0.6 vs. 3.6+/-0.6 mg/dL, p<0.001) and longer LOS than those not at risk (4 days (3,6) vs. 3 days (2,4), p=0.005). FORMS is a quick, valid, and reliable tool to screen adult hospitalized patients for nutrition risk.
