The analysis of hormonal steroids and the ratio of certain steroid metabolites may play a role in the detection of cancers. Eight human urine samples and five environmental water samples were analyzed for the presence of 15 common hormonal steroids. The steroids were extracted and concentrated using a disk solid-phase extraction (SPE) method. Separation was achieved using reversed-phase liquid chromatography (LC) on a C18 silica column. Atmospheric pressure photoionization (APPI) with a toluene dopant was used in both positive- and negative-ion mode to produce [M+H]+ or [M-H]- precursor ions which were quantified by tandem mass spectrometry (MS/MS) through a selective multiple-reaction-monitoring (MRM) transition. The disk SPE-LC-APPI-MS/MS method was validated using percent recoveries of standards in an artificial urine matrix. Disk SPE gave quantitative recovery of 12 of the 15 steroids with recoveries ranging from 82 to 133%. Isotopically labeled internal standards of all 15 steroids were used to account for any sample loss during the sample preparation and analysis.

Analysis of the environmental water samples showed a decrease of testosterone among wastewater treatment samples as the amount of treatment increased. Analysis of the human urine samples followed the expected trends based on typical male and female metabolism. Male samples have higher levels of testosterone compared to female samples, while female samples had more progesterone than male samples. Continued work on this topic could yield a method to detect and quantify these steroids in human urine.
