The purpose of this project was to identify the need and write a grant to fund a mental health outreach program for adolescent gang members and youth who are at risk of joining gangs. The sponsoring agency for the grant proposal was the Transitional Age Youth Academy in Long Beach, California. If funded, the agency would be able to enhance its services by hiring two additional social workers. These outreach social workers will engage youth affiliated with a gang or at risk of joining a gang, assess them for unmet mental health needs, connect the youth with services, and follow up with them regarding their progress. The actual submission of this grant to the California Wellness Foundation was not required to successfully complete this project.
