Diagnosis is a fundamental activity of health care professionals however methods for establishing diagnostic labels that are reliable, valid and meaningful are highly variable. The objective of this study was to take a rigorous methodologic approach to defining diagnostic criteria for carpal tunnel syndrome. The study was conducted in three phases: item generation from a literature search, focus groups of clinicians and key informant interviews; item reduction using a novel panel of experts participating in a Delphi process; weighting and validation of the final list of diagnostic items using clinicians rating a series of clinical scenarios containing the most highly ranked items. The weighted criteria showed a correlation of 0.60 with the opinions of expert diagnosticians.
