Research investigating the neuropsychology of alcoholism has revealed that chronic alcoholics manifest cognitive deficits reflective of cerebral dysfunction. These impairments are often attributed to the direct neurotoxic effects of ethanol on the brain, however, recent evidence has indicated that chronic alcohol abuse may indirectly affect cognitive functioning by disrupting physiological processes outside of the central nervous system. The current studies investigated if alterations in the endocrine system could be contributing to alcoholics' cognitive deficits.Subjects--Topical Terms:

520 $a Study 1 investigated the validity of a new battery of brief and easily-administered cognitive tests, the Cognitive Laterality Battery (CLB), for assessing alcoholics' visuospatial and verbosequential abilities. If found to be valid, this battery could be used to study hormone-performance relationships in alcoholics. Fifty alcoholics and 30 controls were administered the CLB and a flexible battery of verbal and visuospatial tests shown to be sensitive to alcoholics' cognitive deficits. Alcoholics performed significantly poorer than controls on the CLB visuospatial tests, but not on the verbosequential tests. Thus the CLB visuospatial tests are a valid measure of alcoholics' neuropsychological deficits.
520 $a Study 2 investigated the relationships between gonadal hormones and cognitive performance in 58 male detoxified alcoholics and 29 controls. Significant correlations between visuospatial performance and testosterone and follicle stimulating hormone were found in controls but not in alcoholics. Results indicate that alcoholism may disrupt the "normal" relationships between hormones and cognitive performance.
520 $a Studies 3 and 4 investigated the hypothesis that elevated cortisol concentrations characteristic of ethanol ingestion and withdrawal are contributing to alcoholics' cognitive deficits. Alcoholics were administered cognitive tests and interviewed as to the number of annual withdrawals experienced. Cortisol was measured during withdrawal in 18 alcoholics and in response to stress in 52 detoxified alcoholics and 30 controls. Alcoholics showed an attenuated cortisol response to stress indicating a dysregulation in the hypothalamic-pituitary-adrenocortical axis. The cortisol-stress response was negatively correlated to memory performance in controls but not in alcoholics. While cortisol during withdrawal was correlated with poorer problem-solving performance, the number of withdrawals correlated with poorer memory performance. In sum, since cortisol and withdrawal frequency were related to different cognitive deficits, the mechanisms underlying these relationships appear to be independent.
