This is a descriptive and exploratory naturalistic inquiry into the meaning of spirituality using a qualitative research approach. The spiritual dimension of the human being, until recently, has been avoided, misunderstood, or has received little attention in scientific research. A purposive sample was used of African American adults with a reported diagnosis of severe mental illness, who were chosen because they have a rich heritage of religiously expressed spirituality, especially in the face of hardship. The study was conducted with subjects who expressed a belief in God and who were members of a community-based psychiatric rehabilitation organization. This outpatient setting allowed the inclusion of participants who might have been exhibiting fewer actively psychotic symptoms, thus facilitating the interviews. Each subject responded to an in-depth, guided, open-ended, and minimally structured interview; each was asked to share the meaning of their relationship with God.

Four major themes emerged from the data which was coded by use of the computer software, QSR NUD*IST 5 (2000). Those themes were: meanings of God, spiritual needs identification, communication with God, and results of the spiritual relationship on the life. Results of the study contribute to the development of nursing knowledge and theory by describing and defining the meaning of spirituality to those who have experienced psychotic mental illnesses. Nurses can use these results to sensitize themselves to the needs of persons who have strong religiously expressed spirituality which might mediate the negative effects of psychotic mental illness.
