Collaborative systems currently allow physicians to work together over large geographical distances to improve the quality of patient care in remote hospitals and clinics. To be effective much of the medical discussion of patient symptoms, formulation of diagnosis, application of treatment, and therapy, must be performed in real-time using computer assisted collaboration sessions. Real-time medical collaboration requires reliability, Quality of Service, session playback, session logging, and network efficiency. In addition, security is a major issue for confidential patient information transmitted during the collaboration session. Current systems potentially useful for medical collaboration do not ensure the secure transmission of data, the collection of session information for future review, or the efficient use of network resources.

The primary objective of this thesis is to present the design and implementation of the PArallel MEdical COllaboration SYstem (PAMECOSY), a system that allows geographically disperse physicians to reliably perform secure real-time collaborative visualization using an efficient, low-bandwidth network connection that retains a permanent account of the chronological activities of the collaborative session for future review. PAMECOSY is built and tested on the Secure Active VPN Environment (SAVE) test bed, a networking environment that emulates the networking conditions the application is required to perform on.

Tests varying the bandwidth and security available to the system were performed. Results show the PAMECOSY application securely continues a visual collaboration over low bandwidth network connections where other systems fail to operate. Using a two-stage process for visual collaboration presented in this thesis, physicians with a computer, the PAMECOSY application, and phone line are able to visually collaborate securely with peers in other medical institutions.
