Weak visual prestimulation has been shown to facilitate the blink reflex at short and long lead intervals but to inhibit it at intermediate intervals. Blink inhibition at these intermediate lead intervals has generally been attributed to pathways that are entirely subcortical. However, the present study of 13 patients with unilateral visual cortex damage showed that presentation of a prepulse to the blind hemifield was not sufficient to inhibit the blink reflex to cutaneous or acoustic reflexogenic stimuli. The blind hemifield prepulses also failed to facilitate reflexes at short and long lead intervals or to speed voluntary reactions to the reflexogenic stimuli. These results imply that reflex modulation by a visual prepulse is mediated by visual cortex.
