The office-based pediatric medical encounter was the focus of this investigation. A field study with an adjunct survey component targeted four separate, but related components with regard to the patterns of communication and interaction between physicians, preadolescent and early-adolescent children, and their mothers: (a) the identification and comparisons of the children's and the mothers' agendas in pediatric medical encounters, (b) the physicians' patient-centered (child-centered and mother-centered) scores in pediatric medical encounters, (c) the identification and comparisons of the children's and the mothers' respective opinions about the physicians' therapeutic attributes and patient-centered behaviors (child-centered and mother-centered), and (d) the relationship between the physicians' patient-centered (child-centered and mother-centered) scores and the respective children's and mothers' opinions about the physicians' overall therapeutic attributes and patient-centered behaviors.

520 $a The findings indicated that when physicians interact with their pediatric patients, they are actually confronted with a dual patient, or patient dyad, with the child as the primary patient, and the mother as the secondary patient. This study found that the children and the mothers brought distinct personal agendas for the physicians' attention and response. Emanating from the presence of a dual patient were the complexities inherent in triadic communication processes. The children were significantly less likely than the mothers to think that the physicians paid attention to what they were personally thinking and feeling during the medical encounter, and were significantly less likely than the mothers to think that they were provided with adequate opportunities to express their full agendas. The mothers were significantly more likely than the children to think that the physicians paid adequate attention to the children.
520 $a The physicians interacted with their pediatric patient dyads in accordance with the traditional biomedical model, as opposed to the patient-centered model. While the children and the mothers gave high ratings to the physicians for their therapeutic attributes, the children were significantly less generous when rating the physicians' patient-centered behaviors. The study's implications were discussed in terms of medical education, patient education, and the role for professional counselors in these endeavors.
