This study is situated in three general surgery clinics located in two teaching hospitals. This dissertation addresses 15 doctor/patient interactions out of a total of 79 observed. The principal source of data in this study is from detailed observations and tape recordings of doctor/patient interaction.

In this dissertation I observed both similarities and differences between the organizational features of each clinic that influence the doctor/patient interaction. There are differences and similarities in each clinic in terms of: (1) Organizational resources (participants present in clinic); (2) The characteristics and role of participants and their doctor/patient interactions and relationships with other health workers in the organization; (3) The physical environment; (4) The population each served, presenting varying chief complaints and who are provided with different diagnoses and treatment plans; (5) The fee structure; and (6) The policies and procedures.

Not only are there different pattens of doctor/patient interaction that occur across medical organizations, but also different patterns of doctor/patient interaction that appear in the same clinic because patients interact with physicians at different levels of authority. This dissertation provides both examples of differences in doctor discourse versus patient discourse and examples of how each level of authority, attending surgeon, chief resident, resident and medical student carries out communication with the patient. Patients and physicians are seen approaching the exchange with different understandings, attitudes and knowledge.

This dissertation also highlights how organizational aspects can influence the doctor/patient interaction and have strong consequences for the patient while at the same time violating bureaucratic procedures and authority structures.
