Clinical observations show after menopausal women have increased incidence of stroke and Type II diabetes. Diabetes worsens stroke outcome. There are no studies of HRT in postmenopausal diabetic women, a specific at-risk group for stroke. Experimental stroke studies show neuroprotective effect of estrogen non-diabetic females, but it is less clear in diabetic animal.

This study applied the unilateral cerebral hypoxia-ischemia model to female type II diabetic, db/db mice and their non-diabetic littermates, +/? under intact or ovariectomy (OVX) or with estrogen replacement (OVX+E) conditions. Estrogen was protective in +/ ? mice. Db/db mice had more severe brain damage, without any effect of estrogen. Hyperglycemia was then normalized by the oral metformin. Euglycemia only partially decreased brain damage in db/db mice, and again without effect of estrogen. Estrogen brain uptake showed that db/db mice had reduced estrogen brain uptake without effect of blood glucose.

Using RNase protection assay (RPA), present study showed that bcl-2 was induced in OVX +/? mice at 8 hours, with further increase by estrogen replacement. Diabetes completely prevented the induction of bcl-2. Combined with in situ and immunohistochemistry, present study for the first time showed bfl-1 induction in activated microglia/macrophages in ischemic brain. In +/? mice, bfl-1 expression was increased as early as 8 hours, with higher levels in estrogen treated group. In db/db brain, no bfl-1 induction was seen until 48 hours, with no effect of estrogen. The delayed bfl-1 expression was associated with severe brain damage. These data suggested microglial dysfunction in db/db mice, followed by delayed induction of IGF-I, and reduced astrocyte activation.

This is the first systemic study about the effect of estrogen on the Type II diabetic ischemic brain. Data suggest that hormone replacement therapy may not be effective in diabetic stroke even in context of euglycemia, and it may not be due to defect of absolute estrogen brain uptake. Diabetes prevents anti-apoptotic gene expression. Ischemia induces bfl-1 expression in microglia in vivo, and dysfunctional microglia in the diabetic brain may play an important role in stroke.
