We have built a patient simulation system based on the diagnostic expert system, MEDAS, and the medical record system, IMR. It provides a way for an instructor teaching basic sciences in a medical school setting to start with a set of basic science concepts and create a simulated patient with appropriate findings and disorders to help students learn those concepts.

We have built an up-to-date medical record system, the new EAR, to support a variety of research projects, especially the patient simulation system. We decided to construct such a system by combining the SNOMED terminology bank with our old Intelligent Medical Record System. SNOMED includes the International Classification of Diseases with thousands of disorders and very complete lists of medications and procedures, but the old IMR contains the structural organization that we wanted for the record system. The old IMR also has chief complaint and physical examination modules full of valuable information. Our goal was to obtain the best of both.

The core of our system is a Java Dynamic Tree, which contains the knowledge to support the IMR hierarchy, including the Chief Complaints, the Review of Systems, the Patient's Medical History and Surgical History, the Social History, the Physical Examination, Laboratory Tests, Medications, and Special Procedures. This knowledge base is stored in a relational database management system called the Feature Inventory. The Java Dynamic Tree provides the structure for the Patient Database, the Instructor Interface, the Student Interface, and the Disorder Pattern Toolbox.

The simulation system contains both an Instructor Module and a Student Module. The Instructor Module guides the instructor through the patient creation process. It begins by suggesting features and disorders related to given concepts; it provides sets of features consistent with a given disorder; it generates a patient description automatically. Instructors can provide responses to the students, can add their comments, can edit the patient description and can attach multimedia instructional files to the simulated patient. The Student Module presents the patient to the student and provides guidance through the diagnostic process. Furthermore, the performance of the student is recorded and a report is generated.
