Coronary artery disease (CAD) afflicts over 13 million U.S. citizens and was responsible for 7.2 million deaths worldwide in 2001. Identifying CAD in its early stages is vital to improving patient care as risk-factor modification has been shown to significantly reduce coronary events. This SimSET Monte Carlo study examines the feasibility of using positron emission tomography (PET) as a noninvasive means of directly identifying coronary plaque in humans. Given the activity distribution described in this paper, the author determined that to confidently diagnose a single 8 mm3 plaque in the left main coronary artery, an activity concentration seventy times that of the myocardium (1.26x10-5 Ci/cm3) is required if 128x128 images are reconstructed and an activity concentration fifty times that of the myocardium (0.90x10-5 Ci/cm 3) is required if 256x256 images are reconstructed.
