Non-adherence to the recommended medical regimen for patients diagnosed with abnormal Pap smears puts patients with the human Papillomavirus (HPV) at-risk for developing cervical cancer. Young, minority, clinic patients have been identified as those patients at greatest risk for cervical cancer through non-adherence to recommended follow-up medical appointments. The aims of this study were twofold. First, the construct of dispositional monitoring, a psychological coping style for dealing with health threat, was utilized as a means for identifying which patients from a predominantly minority clinic sample are at greatest risk for poor psychological adjustment and non-adherence to follow-up medical appointments. Dispositional high monitors are those patients who attend to potentially threatening information, while low monitors are those patients who avoid threatening information. The second aim of this study was to examine the effect of a telephone counseling intervention on the psychological adjustment and adherence to follow-up appointments for patients with high and low dispositional monitoring styles. Telephone counseling was implemented subsequent to diagnosis but prior to the scheduled follow-up examination appointment. The findings of this empirical study underscore differences in psychological adjustment and adherence to follow-up appointments between dispositional high and low monitors. Specifically, high monitors were found to experience greater intrusive ideation and greater adherence in regard to their colpscopic examination than low monitors. However, the interaction between telephone counseling and dispositional monitoring on adjustment and adherence was non-significant.
