The study was conducted involving 71 low-income mothers having each a child <5 years and who attend the child feeding service, at Bushulo health center in Sidama, southern Ethiopia. Household food production, consumption, child feeding practices, child health, morbidity and mortality data were collected using pretested questionnaire and trained data collectors. Nutritional status indicator measures: height, weight, and mid-arm circumference of children were collected using standard measures and analyzed with Epi-Info, 2000 software program. Based on mother's recommendations of regularly used crops and chemical analysis, two transitional foods were formulated using corn and an endogenous root crop (Enset ventricosum) product called kocho. Results from the baseline data indicated that the main household diet were corn (85%), kocho (76%), kidney bean (44%) but few households raised animals for consumption. Large family size and small land size (97.2%) affected food availability. The prevalence for severe underweight, wasting, and stunting in children were 45%, 4%, and 48% respectively. The low prevalence of wasting could be due to the extra feeding children received at the health center. More than 40% of mothers in this study had experienced the death of a child in the first year of life. Vomiting, diarrhea, respiratory problems, and malaria were common causes of child death indicating underlying nutritional deficiencies. With the transitional food development, corn:kidney bean:pumpkin (CBP) and kocho:kidney bean:pumpkin (KBP) had protein contents of 14.07g/100g (traditional 8.2g/100g) and KBP:13.81g/l00g (traditional 1.45g/100g) respectively on a dry weight basis, with improved amino acid pattern. On a wet-weight basis three large servings per day would be required to supplement breast milk. The presence of pumpkin in CBP and KBP increased the vitamin A value of both mixes. Based of sensory evaluation results the two transitional foods were acceptable to mothers and children. Thus the use of kidney beans and pumpkin could be a potential way to increase the nutritive value of transitional foods served to children in the study area. The severity of childhood malnutrition reflected with severe stunting, morbidity and mortality data suggests the need for community based and child targeted programs in the areas of nutrition and health education, along with strategies to increase household food security of low-income families.
