Today, in the third decade of the HIV epidemic in the United States, rates of infection continue to increase, particularly among minorities, and specifically among inner city substance-abusing African American women with lower levels of income and education. Prevention efforts to date have largely focused on reducing factors influencing substance-abusing African American women's risk of becoming exposed to HIV. The limited success of conventional approaches to reduce substance-abusing African American women's exposure to HIV raises the question as to whether effectiveness in prevention efforts can be increased by directing attention to the protective factors that keep some segments of this population from becoming infected with HIV. The present study sought to provide understandings to address this question.

520 $a Building on current knowledge, the primary objective of this study was to examine the psychosocial and cultural protective and risk factors affecting substance-abusing African American women's susceptibility to HIV. Guided by the ecological approach, social cognitive and gender theories, formative surveys were used to collect data from 150 inner city African American women, ages 25 to 45, who were seeking services at substance abuse treatment facilities. Data analytic strategies included descriptive statistics and correlational and hierarchical regression analyses to elaborate understandings of the relationships among the variables of interest.
520 $a Results indicated that the participants engaged in high rates of HIV sexual risk behaviors, including exchanging sex for drugs and unprotected sex with multiple partners. Correlational analysis revealed that those women who reported higher levels of sexual self-efficacy, condom use self-efficacy, ethnic identity, and peer influence also reported engaging in less risky sexual behaviors. Depression and beliefs about HIV sexual risk behavior were the risk factors that were found to be significantly positively correlated with HIV sexual risk behavior. The regression analysis confirmed that depression, sexual self-efficacy, and condom use self-efficacy are significant independent predictors of HIV sexual risk behaviors. However, mixed support was obtained for the moderating effect of most protective factors, except condom use self-efficacy. These results suggest the need for developing appropriate models to understand substance-abusing African American women's HIV sexual risk behaviors by examining previously unexplored psychosocial and cultural variables.
