This research was a combined in vivo and in vitro study of the integration of factors regulating uterine contractility. We tested the effect of administration of betamethasone, a synthetic glucocorticoid, and nimesulide, a prostaglandin (PG) synthesis inhibitor, into nonpregnant sheep on in vitro myometrial responsiveness. Betamethasone increased spontaneous contactility and the response to PGE

or oxytocin, indicating selective up-regulation of excitatory EP receptors. Nimesulide attenuated spontaneous contractility and sensitivity to oxytocin while increasing sensitivity to PGs, indicating down-regulation of oxytocin receptor and up-regulation of PG receptors.

Comparison of responses of myometrium and mesometrium from ovariectomized, pregnant or parturient ewes revealed an increase in myometrial activity preceding labor, suggesting that ovine myometrial activation occurs by 135 days of gestation; and a greater sensitivity of myometrium than mesometrium to PGs, supporting a major role for intrauterine paracrine factors.

To determine if acute infusions of omega-3 long chain polyunsaturates (n-3 LCPs) would inhibit preterm labor, emulsions of either Intralipid or n-3 LCPs were infused to pregnant ewes and betamethasone to the fetuses. The n-3 LCPs delayed premature delivery, indicating a possible tocolytic value.

Responses of myometrium from the fundus, anterior and posterior corpus and lower segment of the uterus in pregnant baboons showed that only fundus spontaneous contractility correlates with gestational age, implying a pacemaker role. PGE

contracted lower uterine segment. Sodium nitroprusside, forskolin and isoproterenol inhibited contractility in all regions. The response to sulprostone was equal in the four regions. The lower segment might therefore have more inhibitory EP receptors and fewer oxytocin receptors than other regions.

Iloprost, a prostacyclin analog, was shown to inhibit the contractility of lower segment myometrium taken from rhesus monkeys in normal-term or androstenedione-induced preterm labor but not that from monkeys not in labor, suggesting up-regulation of IP receptors and/or down-regulation of EP
