The purpose of this dissertation was to add to the understanding of the use of clinical assessment measures of balance to identify fall risk in community-dwelling older adults. The literature strongly supports balance impairment as an important risk factor for falls, though there are gaps in our understanding of the use of these methods to quantify impairment. A systematic review and meta-analysis of the existing literature was performed evaluating the association between balance impairment and falls in community-dwelling older adults. Prospective data from the Project to Prevent Falls in Veterans was used to evaluate clinical practice guidelines to identifying fall risk; evaluate predictive validity of the Berg Balance Scale (BBS) to identify fall risk and change in balance function due to falling; and to use epidemiological model building to evaluate balance impairment on falling in community-dwelling older adults.

520 $a The systematic review and meta-analysis demonstrated balance impairment on clinical assessment confers a moderate risk for falls. The type of fall outcome, the length of follow-up, and the balance measurement tool impact the magnitude of the association. The most prominent clinical practice guideline's fall risk screening algorithm can identify and stratify risk, but recommendations for the low risk group may not be adequate. Falls among older adults without a recent history of falls were common and factors associated with balance impairment were present in the majority of older adults. Predictive validity of the BBS is superior for multiple falls then either any fall or any injurious fall and use of the scale in a dichotomous format should be discouraged. Balance and functional mobility are negatively affected by recurrent falls independent to other factors associated with functional decline. Lastly, not all methods of evaluating balance impairment are associated with future falls. The number of people identified as having balance impairment varies with the measurement tool used to assess balance. Therefore measurement tools are not interchangeable or equivalent in defining an "at risk" population. The research findings have direct relevance to clinical practice for the use of clinical assessment of balance and the greater public health issue of preventing falls in community-dwelling older adults.
