Pain is a multidimensional construct. One of the factors that may contribute to the experience of pain is an individual's culture. At the present time, few studies have in the pediatric literature have examined cultural differences in pain. The current study sought to add to the pediatric pain literature by examining cultural differences in pain perception in children with polyarticular juvenile rheumatoid arthritis (JRA) in the United States (U.S.) and Egypt.

520 $a Children with polyarticular JRA from Egypt (n = 34) were matched with children from the United States (n = 34). Participants completed three components of the Pediatric Pain Questionnaire (PPQ; Vami & Thompson, 1985): the visual analogue scale (PPQ VAS), the body outline, and the pain descriptor word list. The child's pediatric rheumatologist also rated the child's global disease activity and number of joints with active arthritis.
520 $a The results of the study supported the importance of examining cultural differences in pain expression. Despite similar ratings of disease severity and a greater number of joints with active arthritis, the Egyptian children reported significantly less pain on the PPQ VAS than did the American children; however, the Egyptian children also reported significantly greater pain on the body outline than did the American children. Support for the construct validity of the PPQ VAS in the Egyptian sample was indicted by significant positive correlations between the physician's ratings of global disease activity and number of active joints with the PPQ VAS. The discriminant validity of the body outline was suggested, as there was a lack of significant correlations between the body outline and the PPQ VAS. Children in both countries primarily selected sensory words, which is consistent with previous studies of children with JRA in the U.S. (see Gragg et al., 1996; Varni et al., 1987) and Norway (see Benestad et al., 1996). Differences in the two cultures' belief systems are explored. Limitations and directions for future research are also discussed.
