520 $a Purpose. This study examined the effects of exposure to noise, aminoglycosides, and related factors on the risk of hearing loss in pre-term infants hospitalized in a NICU.
520 $a Background. Hearing loss occurs in 2%--15% of pre-term infants, versus 0.1%--0.3% in full-term infants. It is hypothesized that the combination of ototoxic exposures, such as noise and aminoglycosides, experienced in the NICU may explain much of this incidence.
520 $a Methods. Noise exposure was modeled from measurements specific to the NICU. Exposure information regarding additional risk factors relating to hearing loss was obtained through a medical records review of former NICU patients. Using predicted noise exposure data along with other risk factor information, a model was developed to calculate the probability of hearing loss following NICU hospitalization.
520 $a Results. Pre-term infants born <27 weeks gestational age who required aminoglycosides >7 days while exposed to noise levels produced by mechanical ventilation (>80 dBA) had a 68% probability of hearing loss, increasing to a 79% probability following in utero exposure to cigarette smoke. Regardless of gestational age, infants exposed to aminoglycosides and noise at levels >80 dBA were 14% more likely to fail a hearing screen than were infants exposed to aminoglycosides and noise <80 dBA.
520 $a Conclusions. Exposure to high noise levels commonly measured in NICUs in combination with aminoglycosides increases the risk of hearing loss in pre-term infants, a risk that decreases as noise levels decrease. Thus, application of noise abatement strategies may decrease hearing loss incidence in NICU infants.
