520 $a Objectives. To quantify and describe feeding problems in preterm infants; develop a model of predictive risk factors; and increase understanding of the relationship between maternal depression and feeding interaction and feeding problems.
520 $a Background. Preterm birth sets in motion events which can lead to development of feeding problems. Parents of preterm infants report difficulties ensuring their infants eat enough to grow. Some infants never develop the ability to fully nipple feed. The occurrence of feeding problems impact parents and family because their sole focus becomes ensuring adequate nutrition.
520 $a Methods. First, a retrospective cohort with data from 557 infants born at less than 33 weeks gestation and less than 1500 grams was assembled from the Southern Alberta Perinatal Follow-up Program. Feeding problems during the first 18--24 months adjusted age were analysed using Kaplan Meier Survival Analysis and multivariable Cox Proportional Hazard Modeling. Second, a prospective cohort of 82 infants born less than 33 weeks gestation was recruited from the Calgary Health Region and followed to 1 year adjusted age. Data from chart reviews, maternal interviews, questionnaires and videotapes were analysed descriptively and with univariate Cox Proportional Hazard Modeling.
520 $a Results. The prevalence of feeding problems in the retrospective cohort was 41%; the cumulative incidence was 29.6/1000 person months. Significant predictors were: days on assisted ventilation, small for gestational age status, a history of heart problems, and Grade 3 or 4 Intraventricular Haemorrhage (IVH). Significant correlates were concurrent health issues and neuro-developmental problems. Prevalence in the prospective cohort was 83%; the cumulative incidence was 49.7/1000 person months. Appetite regulation was the most common problem type, followed by oral sensory and oral motor problems. Predictors included: heart surgery, mother infant feeding interaction, maternal emotional lability, and mental confusion; maternal anxiety was a correlate. Appetite regulation was more strongly associated with: surgery, Grade 3 or 4 IVH, heart surgery and age at full nipple feeding while sensory problems were more strongly associated with maternal depression and emotional factors.
