Type 1 diabetes is a chronic illness that requires continuing medical attention, education and self-care. Although college students with diabetes are at risk for improvised diabetes management due to academic and social demands, little research exists on this population. This project was designed in three phases to investigate the attitudes, behaviors, and knowledge of college students with type 1 diabetes while developing and testing an intervention targeted specifically to them. In phase one, focus groups and interviews identified students' pertinent issues and attitudes. Participants acknowledged difficulty in following diet and exercise recommendations, and hypoglycemic reactions were perceived as unavoidable. Barriers to diabetes control included time management difficulties, stress, diet management constraints, inconveniences such as having to test blood glucose and carry supplies, and inadequate finances. Social support issues and motivators were identified, and denial or rebellion surfaced among several students. Phase two involved development and evaluation of a survey instrument guided by the Expanded Health Belief Model (EHBM). Feedback from eighty-three respondents allowed the discovery of numerous inter-relationships among constructs of the model as well as among constructs and behaviors; key variables in outcome achievement included health importance, benefits-costs, locus of control, self-efficacy, and intention. Obstacles to behaviors were identified as situational factors and emotions. Variables predictive of glycemic control included high health importance and benefits-to-costs, and an internal locus of control. In phase three, a diabetes intervention, "Control on Campus", was developed based on the EHBM and Social Learning Theory. The four session program was pilot tested with three groups of students; a fourth group served as the control. Diabetes knowledge and reporting of hemoglobin A1c values improved significantly as a result of the intervention, compared to no increase in the control. Knowledge was positively correlated with several attitudes. In addition, participants reported feeling more support on campus after the intervention, and were more likely to test their blood glucose. Overall, this research yielded substantial insight into characteristics of college students with diabetes. Results of both the qualitative and quantitative investigations should be employed by college health professionals to provide more optimal care for their students with diabetes.
