Health behaviors of homeless person with serious mental illness (SMI) can be understood within the context of the subculture. The health needs of these individuals are often complex because of their mental and physical conditions. Therefore, in order to provide the most effective healthcare for these persons, cultural factors that influence health behavior, such as norms, values, and beliefs, need to be taken into consideration by nurses and other healthcare providers. This exploratory qualitative study sought to describe and understand the subculture of homeless persons with SMI from the perspective of its members using ethnography as the methodology. Sixteen informants with SMI, five women and eleven men, who were homeless at the time of the interview or had been recently homeless were interviewed using a semi-structured format. Five major domains emerged from the data: (a) Being Homeless, (b) Identity, (c) Social Network, (d) Victimization, and (e) Health. These domains were developed through the process of identifying pattern categories and themes. The themes that emerged from the pattern categories were: (a) perception, (b) isolation, (c) separateness, (d) lack of permanence, (e) bodily distress, (f) poverty, (g) different from others, (h) being survivors, (i) subgroups, (j) avoidance, (k) on the alert, (l) situational awareness, (m) superficial association, (n) limiting exposure, (o) separating, (p) bartering, (q) preventing illness, (r) isolating self, and (s) seeking remedy. The five domains, their pattern categories and themes led to the formulation of a theoretical model, which provides a meaningful description of the subculture of homeless persons with SMI. Based on the findings, the following conclusions were drawn: The subculture of homeless persons with SMI can be viewed as a microcosm of the dominant culture. The subculture of homeless persons with SMI is diverse and can be partially explained by the subgroups that comprise it. Members of the subculture of homeless persons with SMI are at high risk of becoming victims of crime. Members of the subculture of homeless persons with SMI primarily use self-management behaviors that are defined within the context of their culture.
