Educational institutions and medical service providers are feeling the push for interdisciplinary training of health care providers. Also referred to as cross-training, this concept challenges professional identity of occupational therapists (OTs) and physical therapists (PTs) in acute and sub-acute medical settings. Before changes in educating OTs and PTs proceed in response to calls for education and service delivery reform, educators and service providers need to be clear about the scope of these professions in theory and practice. Understanding how PTs and OTs view their own and each others' roles and reasoning about clients can help to elucidate pragmatic similarities and differences. Eight hospital-based OT and PT clinicians, and two physicians, were interviewed and observed in team meetings; the OTs and PTs were observed working with patients. In addition, eight college-based OT and PT educators were interviewed and asked to discuss case studies based on patients observed in the hospitals. Data from all interviews and observations were analyzed to answer the following questions: (1) What are OTs', PTs', and physicians' perceptions of OT and PT as part of the health care team? (2) In what ways do OTs and PTs reason about clients' conditions, problems, and treatments? (3) How do OTs and PTs perceive the health care system as influencing their practices and professions? (4) What changes do OTs and PTs believe are necessary to improve the perceptions of their roles by others and the clinical effectiveness of their professional collaborations. (5) On the basis of the above, what are the implication for the future of the two professions in physical medicine settings? OT and PT clinicians shared common perceptions of OT and PT practice, but their knowledge of each others' goals and concerns for specific patients tended to be imprecise. OT and PT clinicians were more similar in their perceptions and clinical reasoning than either was to their respective faculty counterparts. Faculty participants also showed more congruence with each other than with clinicians. These and other findings are discussed in relation to the research questions, methods used, the literature on clinical reasoning and critical thinking, and implications for OT and PT.
