Social workers transitioning into the medical field in rural integrated primary healthcare experience challenges integrated care practice from the perspective of the social worker. The Affordable Care Act transformed healthcare by requiring expansion of services by combining mental health and physical health treatment previously treated separately now practicing collaboratively and collocated. Social workers are practicing collocated in primary medical healthcare using the integrated care model to meet the standards established by the Affordable Care Act. Autoethnographic research allows for the self to be incorporated in the research of the group studied. Through qualitative research, the reader is provided with a closer look at experiences through firsthand, real world reports. This study looks to explore and demonstrate: what challenges social workers face in transitioning to rural integrated care? The social constructivism interpretive framework allows for the views of the participants to lead the reader into understanding how interactions with others impact social work practice and perspective. Eight social workers practicing rural integrated primary care in Tennessee were interviewed, face to face, about experiences within ten topics related to practice: environment, managerial, role, code of ethics, practice, professional relationships, electronic health record, substance abuse, rural, and identity. Results are presented from the perspective of the participants with quotes related directly to research question. Chapters I through III present traditional capstone components of introduction, review of established literature, and research methodology. Chapter IV contains the discussion of findings of mixed results from the eight interviews, quotes from participants, and limitations of research. Chapter V is the principal investigator's personal narrative from integrated care practice and interviews allowing understanding and explanation for the purpose of this research project.
