Premenopausal females can experience fluctuations in endogenous and synthetic sex hormones, estrogen and progesterone. Exposure to fluctuations in endogenous estrogen and progesterone occurs across the menstrual cycle, while exposure to synthetic forms of estrogen and progesterone occur with the use of hormonal contraceptives, such as oral contraceptive pills and intrauterine devices. Endothelial function and muscle sympathetic nerve activity are two important indices of cardiovascular function and health which may be influenced by sex hormones. However, there are still many limitations to our understanding of the impact of menstrual phase and hormonal contraceptives on these key indices of cardiovascular function. To address gaps in the literature, this dissertation examined: 1) the mechanisms influencing endothelial function across the menstrual cycle; 2) the repeatability of the impact of oral contraceptive pill phase on endothelial function across two cycles; 3) the influence of an acute shear stress pattern insult to endothelial function in oral contraceptive pill users; and 4) the impact of muscle sympathetic nerve activity and blood pressure regulation during an exercise stressor in oral contraceptive pill users compared to intrauterine device users. The primary findings of this dissertation were: 1) menstrual phase did not alter endothelial function or estrogen-related proteins; 2) oral contraceptive pill users experienced consistent impairments in endothelial function during periods of synthetic hormone exposure across two cycles at the group level but not at the individual level; 3) an acute shear stress pattern insult impaired endothelial function in oral contraceptive pill users, regardless of pill phase; and 4) muscle sympathetic nerve activity and blood pressure responses to an exercise stressor were not different between two types of hormonal contraceptive users. These findings of this dissertation advance our understanding of the impact of endogenous and synthetic sex hormone exposure via the menstrual cycle and hormonal contraceptive use on fundamental components of cardiovascular physiology and health.
