For nearly half a century, the United States has engaged in the mass prohibition and criminalization of illicit drugs and substances through numerous laws, policies, and practices collectively known as the War on Drugs. Highlighted by deterrence, severe criminal consequences and punishments, the War on Drugs has been highly ineffective, and in some ways counterproductive, in lowering substance misuse and addressing drug-related harms, all while dramatically increasing U.S. incarceration rates and justice system disparities. Researchers, reformers, and advocates have all identified and discussed these many issues in U.S. drug policy and have proposed a potential solution known as the public health perspective. Combining decriminalization with harm reduction, rehabilitation, and prevention, the public health framework emphasizes public health rather than criminalization and punishment. In certain jurisdictions, some changes have already taken place. The present study provides compelling evidence that community coalitions effectively reduce both arrests and incarceration rates, while simultaneously lowering drug overdose deaths and drug-related illnesses-findings that underscore the urgency for significant drug policy reform. These data further indicate that marijuana decriminalization yields more positive social outcomes than full legalization. In contrast, the effectiveness of drug courts, diversion programs, harm reduction initiatives, and rehabilitation facilities showed mixed results, pointing to important areas requiring further investigation.
