In the provision of health care, quality of care has been given importance equal to that of cost containment. Appropriately performed patient surveys allow patients to shed light on the quality of services. Yet, only few studies in the literature have examined patient satisfaction on a global perspective. This study seeks to link the determinants, the dimensions, and the consequence of patient satisfaction. The analysis uses inpatient data collected by the Gallup Organization from five hospitals between January 1990 and December 1990. The data include nineteen satisfaction ratings, one overall satisfaction rating, and one consequence variable. Several patient characteristics, such as age, gender, insurance, length of stay, and service category, are controlled. LInear Structural RELations (LISREL) analysis is adopted to examine the associations among the variables of interest. Two thousand and twenty-seven patients are included in the analysis. The entire patient group is further broken down to a medical patient group, a surgical patient group, and an obstetric patient group. The LISREL measurement model and structural equation model are replicated for each patient group. Most hypotheses are supported and the results of all four patient groups are similar. Patients' satisfaction with the dimensions of art-technical care, physician care, and physical environment are highly correlated. The physical environment has the strongest impact, compared to the other two dimensions, on both patients' overall satisfaction and their intent to recommend the hospital. To the contrary, physician care has little direct influence on patients' overall satisfaction and the intent to recommend. Patients' overall satisfaction, in turn, leads to the intent to recommend the hospital to others. Older patients are more satisfied than younger patients overall for the medical group and the obstetric group. Female medical patients are more satisfied than male medical patients overall. When taking all patients together, male patients are more satisfied. The most surprising finding is that the longer a medical patient stays, the more satisfied the patient feels. Of the four patient groups, the obstetric patient group leaves the most variance unexplained. This study portrays what patient satisfaction is, explains variation in patient satisfaction, and discloses how overall patient satisfaction affects the propensity of patients to recommend the hospital to others. Incorporating an investigation of the effects of patient satisfaction of medical outcomes and patient compliance in future research would enhance the understanding of patient satisfaction as well as improve quality of care.
