Due to emotional turbulence near the end of life, terminally ill patients encounter severe pain, depression, and a deterioration in their quality of life. Loved ones face the process of saying goodbye to the dying person, and many may find it difficult to deal with their grief and loss which may also lead to depression. This study examined how music therapy may improve the depressive symptoms in terminally ill patients and their caregivers. A convenient sample of 30 participants aged 40 and 65 years, including 15 terminally ill patients and 15 caregivers, was recruited from Shalem Hospice Care, a hospice agency in Texas, United States. The Patient Health Questionnaire9 (PHQ-9) self-inventory depression screening tool was used to evaluate the pre- and post-depression scores. A paired sample t-test was used to analyze the pre-PHQ-9 and post- PHQ-9 scores. The results revealed a statistically significant difference in depression scores before and after eight weeks of music therapy for the terminally ill patients: Mbefore = 12.4 (SDbefore = 2.11) and Mafter = 7.2 (SDafter = 2.02), t (20) = 2.05, p 0.03. 95% CI [0.68, 2.74]. The result for the caregivers, though smaller than the patients, was still significant: Mbefore = 6.2 (SDbefore =1.16) and Mafter =5.8 (SDafter = 0.50), t (18) =1.06, p =0.02, 95% CI [0.19, 0.37]. The study found that music therapy can improve depressive symptoms both in terminally ill patients and their caregivers.
