Background. The knee is one of the most commonly injured joints in the body. Many structures of the knee that provide support and stability can be injured. The primary goal of this systematic review (SR) is to compare conservative treatment and surgical approaches for an injury to the anterior cruciate ligament (ACL). Similarly, the critically appraised topic (CAT) is aimed at providing insight into whether conventional treatment or a surgical approach is best for treating individuals with meniscus injury. Methods. A comprehensive search was performed between January 2022 and March 2022. These searches were conducted by four independent reviewers for both the SR and the CAT. Five databases, including PubMed, Medline, Cumulative Index Nursing and Allied Health Literature (CINAHL), and Physiotherapy Evidence Database (PEDro), were utilized. Similar approaches were used in the selection of articles for both the SR and the CAT. The initial screening was performed based on population, intervention, comparison, outcome measurement, inclusion criteria, and exclusion criteria. Once these articles were retrieved, each reviewer screened for duplicates, non-relevant articles, and articles that were of a low level of evidence. An independent reviewer appraised the articles to identify any biases by using the PEDro and National Institute of Health-National Heart, Lung, and Blood Institute tools for the SR and the CAT. Results. Thirteen articles were included for appraisal, which consisted of seven SRs and six randomized control trials. Out of the three articles utilized in the CAT, two were SRs with meta-analyses and one sole SR. Conclusions. Both conservative treatment and surgical treatment are effective in treating ACL injuries. However, there was insufficient data to suggest one over the other in terms of reducing pain. Clinicians should consider the overall patient presentation and severity of ACL damage to make recommendations on a case-by-case basis. The limitations of the SR were that the results are only generalizable to the adult population. Additionally, a wide variety of interventions led to variations in study designs. The quality of evidence was fair to good. As for the CAT, no significant differences were found in comparing conservative and surgical treatment approaches. Conservative treatment should be considered as the first line of intervention before proceeding with surgery.
