In Puerto Rico, the population burden of COPD is difficult to measure, as cases and reported deaths due to COPD are mixed with other respiratory conditions such as Asthma. We profiled the risk and protective factors and behaviors linked to exacerbations in persons with COPD in Puerto Rico and Four Mainland United States Ethnic Groups: Whites, Non-Puerto Rican Hispanics, Blacks and Asians, utilizing the CDC BRFSS. We found that when compared to Whites, Puerto Ricans were at greater risks of COPD exacerbation due to greater odds of having Asthma, being overweight and not exercising. The analyses related to protective factors against COPD exacerbation for Puerto Ricans compared to Whites resulted in mixed findings. The results showed that Puerto Ricans were significantly less likely to be vaccinated against pneumonia compared to Whites. However, Puerto Ricans were more likely to have health coverage and to engage in routine health check-ups than Whites. To alleviate the burdens of COPD on systems and patients, efforts could be directed towards two key areas: a) Evaluating effectiveness of routine assessments of COPD patients to identify and meet the needs of factors that our study found concerning. b) Explore the current landscape of availability of health programs and initiatives directed towards these factors.
