This study investigated the impact of therapists' attachment styles on their patients' perceptions of the therapeutic alliance. There is mounting literature that suggests patterns of attachment are temporally enduring, and exert their effects on interpersonal relationships throughout the lifespan. As a strong therapeutic alliance is perhaps the greatest contributor to a positive therapeutic outcome, the intent of this study was to examine how therapist attachment influences the therapeutic relationship. Thirty-six therapist-patient dyads participated in the study. Twenty-six psychiatric residents were asked to complete the Attachment Style Inventory (ASI), Hazan and Shaver's attachment self-report (HS), and the therapist version of the Penn Helping Alliance Questionnaire (HAq-I). Thirty-six of their patients completed the Symptom Checklist-90-Revised at the beginning of treatment and the patient version of the Penn Helping Alliance Questionnaire (HAq-I) following the third session, and again, after three months of treatment. Results indicated that therapist attachment as measured by the HS was not significantly related to patients' alliance ratings; however, on the ASI therapists who reported a greater hostile style of attachment received lower alliance ratings from their patients. Additional analyses identified significant relationships between alliance ratings and depressive psychopathology, and correlations between therapist age, birth order and attachment security. These findings are discussed with emphasis on their therapeutic implications and directions for future investigation.
