The Taiwanese government implemented Global Budgeting (GB) in July, 1998, to control growing medical expenditures and decrease the deficits resulting from the previously used programs under National Health Insurance (NHI). Dentists were the first group managed under this new health insurance system because dental care claims were simpler than claims of other specialties.

520 $a The purpose of this study was to evaluate how GB has affected dental service fees and the utilization of dental care. To examine the dental service fees, this study measured the changes of overall reimbursement and service charges at the practice of each dentist. To evaluate how dental utilization changed, this study measured the number of visits for three high reimbursement procedures and two low reimbursement procedures.
520 $a Most data for this study came from the reimbursement and utilization data for dental services during 1996 to 2001. They included the number of visits for different cases, claimed fees, materials used in different procedures, the number of procedures performed in dental care institutions and different BNHI regions of all contracted providers. Descriptive statistics, ANOVA and time series analysis were used to describe how GB has directed its funds and how dentists changed which procedures they preferred to perform after GB. Multiple regression analysis also reflected how the rate of growth for different dental procedures had changed after the implementation of global budgeting. The results show that the reimbursement per visit increased significantly after GB, which suggests that dentists tended to perform procedures that consumed less time and to avoid time-consuming procedures. Preference also varied among the BNHI regions and the different types of dental institutions. Based on the results of this study, it appears that the BNHI and Taiwan Dental Association should regulate payments for dental procedures to help dentists perform optimally and to ensure that patients get comprehensive health care. Suggestions for implementation and further study of GB are discussed in the final chapter.
