State and local governments have experimented in recent years with tax-exemption policies in an effort to influence the actions of nonprofit hospitals. This study compares how approaches to tax-exemption policy in Pennsylvania and California are affecting the behavior of nonprofit hospitals within these states regarding the provision of charity care and community benefits.

520 $a The examination is supported by research into two interrelated problem areas. The first is to determine how legislation designed to regulate nonprofit hospitals through their tax-exempt status has and can influence their behavior regarding provision of these charitable services. The second is to determine whether the organizational structure of the nonprofit hospital is sufficiently responsive to economic and societal expectations and demands to retain viability within the United States health care marketplace.
520 $a A central thesis of this study is that public policy efforts to reinforce or encourage nonprofit hospitals' traditional role of providing charitable services as a quid-pro-quo of tax-exemption and the health care industry's promotion of community health care management as the nonprofit hospital's primary role are means to the same end: that of improving community health status. In both Pennsylvania and California the increased attention of public policymakers to nonprofit hospitals' tax-exempt status played a role in bringing charity care and community benefits to center stage in hospitals' organizational thinking. It contributed to renewed contemplation about the responsibilities of nonprofit hospitals in contemporary society. Both states enacted legislation which potentially could open doors to greater public-private sector collaboration in exploring opportunities for partnerships.
520 $a Nonprofit hospitals fill a distinctive niche. They enhance consumer choice, because they provide some services which are not self-sustaining and which address health and social service needs of entire communities. Nonprofit hospitals, as quasi-public institutions, and state governments have a shared responsibility and a shared potential benefit in demonstrating that they are accountable to the public trust.
