Posttraumatic stress disorder (PTSD) and trauma-related symptoms have been shown to be associated with physical problems and increased medical utilization. In addition, anger has been found to play an important role in PTSD, and has also been linked, independent of PTSD diagnosis, to physical health problems. Specifically, both anger expression and anger suppression have been found to be positively associated with physical health problems. Despite a large literature addressing various aspects of PTSD, anger, health problems, and medical utilization, a clear understanding of the interplay of these variables remains elusive. This cross-sectional study attempted to clarify these relationships by examining (1) whether anger expression and anger suppression mediate the relationship between PTSD symptoms and physical health problems and (2) whether PTSD symptoms are directly associated with medical utilization. One hundred and ninety nine primary care patients from an outpatient clinic in the VA San Diego Healthcare System participated in the study. Participants were recruited during a primary care visit. Before their appointment, they completed a questionnaire battery, which included demographic questions, measures of PTSD, anger, physical health, and medical utilization. In addition to these self-report measures, physical health and medical utilization information was extracted from the participants' medical charts. It was hypothesized that (1) anger expression and anger suppression would mediate the relationship between PTSD symptoms and physical health problems and (2) PTSD symptoms would exert a direct influence on medical utilization.Subjects--Topical Terms:

520 $a Structural equation modeling (SEM) was used to examine the study hypotheses. Results indicated that higher levels of PTSD symptoms and anger suppression scores were associated with a greater number of physical problems. However, the hypothesized mediating effects of anger expression and anger suppression on the relationship between PTSD symptoms and physical health problems were not supported by these analyses. In addition, PTSD symptoms were positively correlated with medical utilization. However, rather than finding a direct association between PTSD symptoms and medical utilization, physical symptoms accounted for this relationship. These cross sectional associations are suggestive of a possible effect of PTSD on physical health. Implications and future directions are discussed.
