Parasympathetic (vagal) stimulation shifts the site of initial activation of the right atrial epicardium from the SA node in the rostral sulcus terminalis in the high right atrium to the caudal sulcus terminalis region in the low right atrium. Such a change in atrial activation may also change the activation of the atrioventricular (AV) node and affect atrioventricular conduction. The objective of this project was to determine whether vagal stimulation affects the activation times of the atrial inputs to the AV node, and to determine whether the vagal effect on atrioventricular conduction depends on such changes in activation sequence.

520 $a Before and after we stimulated the vagus nerves, we recorded from the anterior and posterior atrial pathways that conduct impulses to the AV node. During the first cycle that was prolonged by vagal stimulation, the elapsed time between the activation of posterior and anterior pathways decreased significantly, and the change in the posterior-to-anterior activation time was usually preceded by a pacemaker shift toward the inferior sulcus terminalis region. This suggests that vagal stimulation alters the AV nodal activation sequence, and that this alteration is probably achieved by a pacemaker shift to an extranodal site.
520 $a To determine whether such vagally-mediated changes in the AV nodal activation sequence affect the A-V conduction time, we paced at sites in the anterior and posterior regions in the right atrium, and we measured A-V intervals after vagal stimulation. Vagal stimulation prolonged the atrioventricular conduction time by up to 30 ms more when we paced at the anterior region than when we paced at the posterior region. Changes in pacing site affected the A-V interval most when we stimulated the vagus at those times that caused the A-V intervals to prolong maximally. These results support the hypothesis that the sequence of activation of the atrial inputs to the AV node modulates the effect of vagal stimulation on A-V conduction. Such evidence of vagally-mediated conduction changes suggests that the parasympathetic nervous system can increase the dispersion of refractoriness throughout the atrium and the AV node, and increase the susceptibility to AV nodal reentry.
